New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. 

You will need to complete a GMS1 and the relevant form below, then hand in to reception in person (if over 18). These forms will provide useful information whilst we wait for your medical records to arrive from your previous doctor. 

New patients are obliged to have a health check within 28 days of registering. This helps us to get to know you prior to your notes arriving from your previous doctor and to determine any care that you may need, in particular those on regular medications.

Medical treatment is available from the date of registration. Please contact reception for further information.
